Tito from Macho Nacho Productions has posted a video showing off NESCD, a hardware project that adds CD-ROM support to the original NES. The system is built around a custom board using an RP2350B chip, which connects to an IDE CD-ROM drive as well as an Everdrive's USB developer port. This setup allows patched games to be run from CD-ROM on original NES hardware.
